Math please help looked all over the book but still don't know how to do it?

For the first one, the only trick is to read carefully and notice that the health premium is given as a monthly rate, and the other others are yearly rates. To get a monthly rate, divide the yearly rate by 12. So for example, if the yearly rate was $240, then the monthly would be 240/12 = $20 per month. Find the monthly rates for both the dental and vision, and then add the numbers for health, dental, and vision.
