what u do is u go into the office with ur proof of income ,residence,License,ID,social security card and when u have the baby a birth certificate.when u call to make an appointment just ask them what u need they will tell u and they give u the vouchers in the office. It's not as bad as u think just try it ..it really helps and if u decide to stop feeding they will provide formula for u just call them and ask for formula when u stop.
